About the role

East Sussex County Council has one of the largest integrated health and social care programmes in the UK, The East Sussex Better Together Alliance which is an ambitious project to integrate the local Authority social care systems with health services in the County to provide better outcomes for residents.

East Sussex County Council are looking to identify an outstanding candidate to take on the position of Head of Care Management (Hospitals). This position will help deliver first class social care services as part of a supportive and innovative leadership team who put patients at the heart of everything.

Key Responsibilities

• Responsible for the operational and strategic leadership of the service.
• Build strong and productive relationships with all Health Services and other partners and represent East Sussex County Council at strategic System meetings.
• Lead on statutory adult social care responsibilities for acute and community hospital services including advising on all issues relating to the Care Act, Mental Capacity Act and Adult Safeguarding.
• Representative on local A&E Delivery Boards and forums associated with hospital discharges, for example: Combined Urgent Care Operational Resilience Group, Winter Delivery team, NHSE Winter debriefing workshops.
• Member of the East Sussex Better Together Operational Executive.

About you

Your working knowledge of relevant legislation and care pathways will help you navigate the territory but it will be your ability to manage relationships between the local authority and NHS that will allow you to thrive in this role. You will bring significant social care leadership experience and the ability to apply critical thinking to social care system design while championing outcomes for the residents of East Sussex.
